New Domestic Violence Protection Orders

New Domestic Violence Protection Orders went into effect April 15, 2021. These forms were last updated in 2014. If you have the old forms in your system, you will need to replace them. The new forms are available in Adobe PDF or Microsoft Word format, and in languages other than English, on the Supreme Court of Ohio Domestic Violence Program website.

Here’s what you need to know:

 

Domestic Violence Protection Order Forms

There are 10 NEW forms that include plain language and guidance for victims to give the court better information to help courts improve their orders and reduce continuances.

Changes in the forms:

    • Address when protection orders are entered into the FBI National Database for Firearms Disability and Protection Orders. See Form10-A and Form 10-B
    • Provide forms that prove offenders know about the orders and may be subject to arrest if violated. See Form 10-C and Form 10-D
    • Notify the Court upon receiving firearms for protective custody, See Form 10-F, and Notice Concerning Possession or Purchase of Firearms. See Form 10.04-A
    • Add dating violence protection order forms to comply with new legislation. See NEW Forms 10.01-P, 10.01-Q, 10.01-R, 10.01-S, 10.01-T.

 

Juvenile Civil Protection Order Forms

Changes in the forms:

    • Address when protection orders are entered into the FBI National Database for Firearms Disability and Protection Orders. See Form10-A and Form 10-B
    • Amend the form on How to Complete a Petition for a Juvenile Civil Protection Order or a Juvenile Domestic Violence Civil Protection Order. See Form 10.05-A
    • Update the Petition for Juvenile Civil Protection Order and Juvenile Domestic Violence Protection Order. See form 10.05-B.
    • Revise the Juvenile Civil Protection Order or Juvenile Domestic Violence Civil Protection Order Ex Parte, See Form 10.05-C, Juvenile Civil Protection Order Full Hearing, See Form 10.05-D, and Juvenile Domestic Violence Civil Protection Order and Consent Agreement Civil Protection Order. See Form 10.05-E.

 

Stalking and Sexually Oriented Offenses Protection Order Forms

Changes in the forms:

    • Address when protection orders are entered into the FBI National Database for Firearms Disability and Protection Orders. See Form10-A and Form 10-B
    • Amend the Criminal Protection Order See Form 10.03-B
    • Update both Ex Parte and Full Hearing forms for Civil Stalking Protection Order or Civil Sexually Oriented Offense Protection Order, See Forms 10.03-E and 10.03-F, and Instructions for Obtaining Civil Stalking Protection Order or Civil Sexually Oriented Offense Protective Order. See Form 10.02-G
